Electronic Records & Signature Disclosure

This disclosure ("Disclosure") is provided to you by eSign Services in compliance with the U.S. Electronic Signatures in Global and National Commerce Act (15 U.S.C. §§ 7001 et seq., the "ESIGN Act") and the Uniform Electronic Transactions Act ("UETA") as adopted in your state. Please read it carefully before signing any document through the Service.
1. Consent to use electronic records and signatures
By clicking "Sign document" or otherwise affirmatively indicating your agreement, you consent to:
- Conduct business with the sender of the document electronically;
- Use an electronic signature in place of a handwritten signature; and
- Receive the signed document, the audit trail, and any related disclosures electronically.
2. Hardware and software requirements
To access and retain electronic records you will need:
- A current web browser (latest two major versions of Chrome, Safari, Firefox, or Edge);
- A valid email address that you can access;
- Sufficient storage to save, or a printer to print, PDF documents; and
- A PDF reader (most modern browsers include one).
3. Withdrawing consent
You may withdraw your consent to receive electronic records at any time before signing by closing the signing window without clicking "Sign document," or, after signing, by contacting the document sender directly. Withdrawing consent will not affect the legal validity of records already signed. If you withdraw consent before signing, the document sender may need to arrange an alternative (e.g. a paper signature) to complete the transaction.
4. Paper copies
After signing, you can download and print a PDF copy of the executed document — including the appended Signature Certificate — directly from the signing confirmation page or from the email receipt sent to you. eSign Services does not charge for paper copies; you are responsible for any printing costs.
5. Updating contact information
To update the email address used to deliver electronic records for a particular document, contact the sender of that document. To update contact information held by eSign Services for your sending account (if any), email james.allyn@yahoo.com.
6. What we record
For each signature event, eSign Services records the signer's name, email address, IP address, user agent, and timestamps. This information is included in a tamper-evident Signature Certificate appended to the signed PDF and stored as part of the document's audit trail.
7. Acknowledgment
By proceeding to sign, you confirm that you can access this Disclosure and the document presented to you, that you meet the hardware and software requirements above, and that you consent to use electronic records and signatures for this transaction.
